講演概要

Evidence Based Medicine posits that we must use the best available evidence to guide the care of our patients. But what if the best available has methodological flaws? The biggest challenge with finding “real” answers to the questions about bone and joint infections is simply the lack of high quality Evidence. What is high quality evidence? It is evidence derived from the design and conduct of large, randomized trials. What is large? In most instances sample sizes greater than 1000 patients (according to Richard Peto and Salim Yusuf-pioneers in clinical trials) is sufficiently large, but more recently PJ Devereaux (Surgeon scientist, McMaster University) has argued that 1000 patients may be too small.

Join Dr. Bhandari as he provides answers many important questions for the future of bone and joint infection research? These include: 1. Why are small studies dangerous?, 2. When is study big enough?  3. How should we design and execute large trials in infection? and 4. The characteristics of the scientists who are needed to move the field forward in the future.

Using examples from the infection literature, as well as trials conducted by Dr. Bhandari himself, he will engage this discussion in a simple, and clear format. There will be no complicated formulas, no sample size calculations—just critically important principles that every student, scientist, surgeon, and physician must know.

The future of research depends on a new generation of scientists with creativity and resilience. Large datasets will become the norm—and understanding why we need to think even bigger is the challenge!
